If a student is not present after 15 minutes from scheduled start time, this constitutes a “no show” and will be a subject to a $35 rescheduling fee. Students arriving for in- vehicle lessons without their permit will result in a cancelled lesson and rescheduling fee.
Students must have the learners permit with them at all times when driving or observing in the car. Students must wear glasses or contacts during in-vehicle lessons if they are required. Students are required to wear closed-toe shoes (no sandals/ flip-flops/heels) during lessons for safety reasons.
If there is a cash Pick up/Drop off fee, it is due on first lesson or paid along with the invoice (fee based on student's location).
No show to lesson, no permit or less than 24hr notice of cancellation may result with a fee of $35.
If at fault, the students are responsible for any speeding violation tickets acquired during their lesson.
The Instructor reserves a right to Cancel/Reschedule lessons due to; safety concerns, dangerous weather, unsafe road conditions, vehicle malfunction, illness or personal emergency.
Unused Lessons Expire after 60 days from the first session.
The school will not refund any tuition or part of tuition if the school is capable and willing to perform its part of the contract.
All disputes under this Section shall be directed to the Secretary of State's Office

I am the guardian/parent of the enrolling student and give my legal consent for him/her to participate in this Traffic Safety Education program. I have read the fees and fine print and will be responsible for the payment of all tuition and fees due.
Before instruction occurs, the Parent/Guardian or Student must notify the School of any health or medical condition or medication that could impair the Student’s ability to operate a motor vehicle. The condition or medication must be clearly defined.
The School reserves the right to reschedule any lesson without prior notice should the instructor believe the Student’s physical or mental condition is not conducive to safe, proper vehicle operation.
Teen Galaxy Driving School Inc. Policies & Contract DOCUMENTS NEEDED TO OBTAIN LEARNER'S PERMIT AT DMV Students must bring the following Original forms when applying for their Learners Permit at the DMV: Original Birth Certificate or Valid Passport if born outside the U.S. Current Illinois State ID or Student School ID Original Social Security Card Proof of Residency.- 2 pieces of mail addressed to the student from their high school Permit Application(student must be enrolled in an approved drivers education program and obtain a permit application from a certified driver education school such as Galaxy driving School) Students must Pass a vision and a written knowledge test at the Secretary of State. Pay $30 Application Fee (Cash or Check payable to the IL Secretary of State ) The Permit allows the student to drive while supervised by a parent or adult(21+), it is valid for two years. It must be held a minimum of nine months and must not get any convictions. The Teen must bring it to ALL practice driving or BTW training sessions. B. CLASSROOM TRAINING A student must be 15 years old by the last class session to enroll in a class. The Classroom is located at 6105 W. Addison. Our Teen Program consists of the Illinois mandatory 30 hours of classroom(15 days/2 hours per day). Students can miss up to 4 classroom sessions but each session must be made up(students must contact the instructor for available make-up days). During class sessions students will be given documents enabling them to take the Instruction Permit Test at the Secretary of State's office. Parents are responsible for getting their teen to the Secretary of State’s office to take their permit test as well as making sure that students are on time to driving & classroom sessions and get picked up immediately after. Once the student receives his/her Learners Permit, he/she will be scheduled for a behind the wheel instruction; and observation if signed up with a partner. A copy of students' learners permit must also be given to the office for record. After receiving the instruction permit, students are still required to complete the classroom portion of the course .If the student drops out of the course, the instruction permit will be canceled by the school which initiated its issuance. Students are to conduct themselves in a respectable manner, just like at their school. Any damage or graffiti to any school property, books or vehicles will result with a replacement of the damaged item and a fee. Parents will be notified the 1st time a student fails to follow classroom behavior rules subsequent violations mainly to expulsion which will result in a forfeiture of all amounts paid to date and cancellation of the students learners permit with no exceptions. A minimum grade of 70% is required to satisfactorily pass the classroom phase. Students who do not meet this requirement must pay a reinstatement fee and repeat the classroom training. Classroom portion will have a Final Test at the end of the program. Any classroom changes requested by a customer such as, but not limited to, transfers, changing start date and changing classroom time after the third day of the class session are subject to a $200 nonrefundable fee. Price is not negotiable. C. DRIVING SESSIONS Once a student receives their learner's permit and completes at least 4 hours of classroom, driving lessons can be scheduled. Behind the wheel sessions are by appointment and can be flexible to fit students' schedules and are on a first come, first serve basis. A student may be driving in the vehicle with the instructor by him/herself or there may be up to 2 other students in the vehicle depending on the option purchased. The student must bring their Learners Permit to All behind-the-wheel driving sessions. Behind-the-wheel instruction lessons must be completed within 3 months from the first practice driving session. Failure to do so may result in an additional fee. According to Illinois state law, students MUST possess their ORIGINAL instructors permit for all In-Vehicle lessons while lessons are being conducted. A copy or image of the permit is not acceptable. Students must wear glasses or contacts during in-vehicle lessons if they are required. Closed toe shoes are required (no sandals/flip-flops/heels) during lessons for safety reasons. Pick up locations may vary and there may be an extra charge based on the package purchased and students location (distance perimeters apply). Home & School pick up/drop off is available! NOTE: All courses enrolled for at Galaxy Driving School must be completed within 9 months or there will be a re-enrollment fee of $425. It is the student and parents’ responsibility to make time for these lessons as it is not the instructors job to keep pursuing their student. You MUST make time for these driving hours. The earlier the appointment is made, the earlier the student drives. It is best not to wait 9 months to barely start as there might not be any spots to accommodate the student. All In-Vehicle training must be completed within 9 months from the first day of classroom (the “Completion Date”). This is a Secretary of State requirement. In-Vehicle training scheduled after the Completion Date will be subject to an additional $425 fee. Students MUST pass an In-Vehicle evaluation in order to be eligible for the Secretary of State Certificate of Completion. * 30HRS Classroom/6 HRS BTW/6 HRS OBS. Basic Driver Program 12 hours in-vehicle total requires a partner(not provided) 4- 3 hour sessions of behind the wheel training ; 1.5 hours driving and 1.5 hours observation each session 30HRS Classroom/8 HRS BTW. Note: If needed, the instructor may take a fifteen-minute break. ⏳ Teen Program Completion Timeline Policy To ensure all students receive timely instruction and to maintain scheduling availability for new enrollees, we require the following steps be completed within a specific timeframe: - Permit Requirement: Students must obtain their learner’s permit within 90 days of completing the classroom portion of the teen program. - Scheduling Behind-the-Wheel Lessons: The first behind-the-wheel session must be scheduled within 120 days of classroom completion. This ensures students begin their driving instruction while the material is fresh and while space is available. - Re-enrollment After Delay: Students who do not obtain a permit or schedule their first driving session within the above timeframes will be required to re-enroll at current rates and based on instructor availability. Priority is given to new students actively progressing through the program. We understand that delays can happen, but due to high demand and limited instructor availability, we cannot guarantee placement for students returning after extended gaps. We encourage families to plan ahead and stay on track to ensure a smooth and successful experience. D. GENERAL -Permit applications will be made available following the first week of classes, contingent upon the submission of a signed counselor form confirming that the student is in good academic standing, or a transcript demonstrating that the student has successfully completed eight courses over the past two semesters. Furthermore, it is essential that any outstanding tuition balance is settled prior to the issuance of the permit. -Parents bear the responsibility for the timely drop off and pick up of students for class and in- vehicle Lessons at the pick up and drop off locations designated by Galaxy driving school. F. COMPLETION All students that successfully complete the program will earn the IL State Certification. If the student’s high school requires driver's education for graduation this certification will satisfy the requirement. In order to receive the certificate of completion from Galaxy driving school the student must: -Complete all classroom (30 Hours), Driving and Make-up Sessions -Submit a drivers education approval form(counselor form) or grades(Official Transcript) -Not have any outstanding tuition balance G. STATE REQUIREMENTS FOR TEEN STUDENT TO OBTAIN DRIVER'S LICENSE: In order to obtain a driver’s license, the state of Illinois requires every teen to : Complete full 30 hours in-class instruction with 6 hours of one-on-one behind-the-wheel training and 6 hours observing a teen partner behind the wheel, OR Complete full 30 hours instruction with 8 hours of one-on-one behind-the-wheel training. Teen must be allowed by Parent/Guardian to be enrolled in Teen Driver's Education Program and the accredited school must enter a certificate of Completion in the system. Student must hold the Learners Permit for a minimum of 9 months Must be 16 years old. Log 50 hours of additional practice(10 hours at night)BTW with parent/guardian or instructor. Must bring Report Card, Counselor Form, or Transcript proving they passed the last two semesters( 8 Classes Total) Pass a final DMV driving test in the vehicle provided by the student or Galaxy D.S.(DMV option) that is legally equipped , in safe/clean condition and properly registered and insured.
